Change of Records Form

Fill out this form to update your name, address, phone number or email address with the Medical Marijuana Program. Any changes to the information provided on your application should be sent to the State within 5 business days. Please be sure to read the document carefully and submit the necessary documentation and fees for name changes and address changes.

Lost, Stolen, or Damaged Card

If you have a Lost, Stolen or Damaged Card, please fill out and mail this form to the Medical Marijuana Program with a check or money order made out to, “Treasurer, State of CT.”

Change of Dispensary Facility Form

Fill out this form to begin the process of changing your dispensary. Changing your dispensary facility can take up to 48 business hours. Please contact us for more information.

Medical Marijuana Brand Registry Database

The Department of Consumer Protection (DCP) announced that a Medical Marijuana Brand Database is now available to the public through Connecticut Open Data. The Database will include medical marijuana brands registered with the DCP complete with name, manufacturer, image of the label, image of the medication, dosage type and potency.
